AGREEMENT
I agree to be bound by the terms of the Constitution and the Member Protection Policy of North's Cricket Club Inc.
UNDER 18’s INDEMNITY AND RELEASE
I/We agree to my/our child applying to and being allowed to participate in cricket matches, training and associated events (“Activities”) organised and/or supervised by the Club and Association. In consideration of the Club and Association allowing my/our child to take part in the activities. I/We acknowledge, agree and confirm the following:
a) That there are inherent risks associated with the activities which may result in my/our child being injured in a serious manner. I/We fully accept and agree to bear those risks in our own right and on behalf of my/our child.
b) To the full extent permitted by law, I/we agree both on behalf of my/our child and in my/our respective rights to absolve, indemnify, release, and discharge the Club and Association, its officers, employees, representatives, and agents (“Indemnitees”) from any and all liability from any injury, loss. cost, charge, expense or damage suffered by me/us or my/our child however caused, arising from or incurred directly or indirectly as a result of my/our child’s participation in the activities, including without limitation as a result of any act, default, omission or negligence of the indemnities.
I/we have read, understood, acknowledge and agree to all the matters referred to in the statement, including the warning, release, and indemnity.
CONSENT TO DISCLOSURE PRIVACY ACT
I/we as the parents or legal guardians of the said child hereby agree and consent to the provision of the personal information regarding my/our child as set out in this form to the Queensland Cricket Association Limited for use by it as it sees fit in the course of its Administration of Cricket in Queensland.

MEDIA CONSENT
On behalf of the individual identified above, the person or persons signing this Registration Form grants permission for Norths Cricket Club to use and retain the individuals;
- Name, image and other identifying information as set out in our Clubs Member Protection Policy, which is available on our Website at http://www.northscricket.com.au

a) There are inherent risks associated with the activities which may result in personal injured (even of a serious nature) to participants. I
fully accept and agree to bear those risks.
b) To the full extent permitted by law, I absolve, indemnify, release, and discharge the Club and Association, its officers, employees, representatives, and agents (“Indemnitees”) from any and all liability from any injury, loss. cost, charge, expense or damage suffered by me, however caused, arising out of my participation in the activities, including without limitation as a result of any act, default, omission or negligence of the indemnities.
I have read, understood, acknowledge and agree to all the matters referred to in the statement, including the warning, release, and indemnity.
CONSENT TO DISCLOSURE PRIVACY ACT
I acknowledge and agree that the personal information relating to me as set out in this form, including without limitation my name and address, may be provided to the Queensland Cricket Association Limited and their affiliates, for use by these organisations, as they see fit in the course of its Administration of Cricket in Queensland.
